The following fees are estimates of POSITIVE CONNECTIONS PLUS, LLC\u2019s normal and customary charges for services. If you receive any other services during your appointment, you will be charged accordingly. You will only be charged for those services that you receive. All amounts are subject to change with or without notice.<\/p>\n\n\n\n

No one will be denied access to services due to inability to pay, and there is a discounted\/sliding fee schedule available based on family size and income.<\/strong><\/p>\n\n\n\n

Clinical & Community Based<\/u><\/strong><\/summary>
\n

$280 per each Mental Health and\/or Anger Assessment. The fee for intake and testing includes scoring and report writing time.<\/p>\n\n\n\n

$168 for individual counseling (45-50 minute session).<\/p>\n\n\n\n

$126 for individual counseling (30 minute session).<\/p>\n\n\n\n

$84 per hour for group session.<\/p>\n\n\n\n

$210 for family and relationship counseling.<\/p>\n\n\n\n

$140 per hour of Skills Building (CBRS), Targeted Care Coordination, Case Management, or Peer Support.<\/p>\n\n\n\n

$32 per hour of Skills Training and Development (STAD).<\/p>\n\n\n\n

$56 per hour of Respite Care.<\/p>\n<\/div><\/details>\n\n\n\n

Psychological\/Neuropsychological Evaluation<\/u><\/strong><\/summary>
\n

$168 for initial appointment.<\/p>\n\n\n\n

$141 per hour of testing.<\/p>\n\n\n\n

$156 per hour of scoring and interpretation.<\/p>\n\n\n\n

$189 for feedback session.<\/p>\n<\/div><\/details>\n\n\n\n

Medical<\/u><\/strong><\/summary>
\n

$364.00 for Initial Visit and Medical Examination.<\/p>\n\n\n\n

$168.00 for follow up appointments. $126.00 if cash payment paid date of service.<\/p>\n\n\n\n

Other charges may be assessed for additional procedures or testing.<\/p>\n<\/div><\/details>\n\n\n\n

Litigation Costs<\/u><\/strong><\/strong><\/summary>
\n

$140 per hour for court costs due to professional time and lost wages.<\/p>\n\n\n\n

$70 per hour for report writing.<\/p>\n\n\n\n

Other charges may be assessed for any related costs.<\/p>\n<\/div><\/details>\n\n\n\n

Provision of electronic Personal Health Information (ePHI)<\/u><\/strong><\/summary>
\n

$0 per request for coordination of treatment or to schools.<\/p>\n\n\n\n

$15 per request for Social Security.<\/p>\n\n\n\n

$30 per request for all other purposes.<\/p>\n<\/div><\/details>\n\n\n\n

Non Medicaid\/Medicare clients may be assessed a $45 fee for No Call\/No Show.<\/p>\n\n\n\n

THERE WILL BE A $35 FEE FOR ANY ELECTRONIC OR PAPER CHECKS RETURNED FOR ANY REASON.<\/p>\n","protected":false},"excerpt":{"rendered":"
