It is required that you bring your insurance card to your appointments.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Insurance organization\u2019s name and address<\/li>\n\n\n\n<li>Your policy and group numbers<\/li>\n\n\n\n<li>Policyholder\u2019s name and employer<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">To make a payment on your account:<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You can call the front office with your credit card information.<\/li>\n\n\n\n<li>You can come to the office and pay with a credit card, cash, or check.<\/li>\n\n\n\n<li>You can mail a check with the client\u2019s name.<\/li>\n\n\n\n<li>You can Venmo to Positive Connections Plus, LLC @PosCnx2023 (Please include client name and be sure to set transaction to private)<\/li>\n\n\n\n<li>You can pay a balance in the Patient Portal<\/li>\n<\/ul>\n\n\n\n<p>\u00a0<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">NOTICE TO PATIENTS:<br>This practice serves all patients regardless of inability to pay.
