Job Title: Targeted Care Coordinator (TCC)
Reports To: Community Coordinator
Summary: The focus of the TCC is to provide coordination services to families who are YES Program approved, or who access children’s behavioral health services through Positive Connections Plus, LLC. The Targeted Care Coordinator is a master’s or bachelor’s level employee. They facilitate the intake process for all children with Medicaid. They work in conjunction with the Recovery and Resiliency (R&R) Leads, client parents/guardians, as well as community providers, to link the individual to needed services.
Essential Duties:
- Follow YES Program and OPTUM regulations as outlined in the Targeted Care Coordination Toolkit.
- Make initial contact(s) and maintain contact with youth and family (telephonic or in-person) at least one time monthly.
- Coordinate and lead Child and Family Team meetings.
- Complete and submit Person Centered Service Plans (PCSP) for each child.
- Gather necessary treatment documents, (e.g., CDA, CANS, psychological testing, current treatment plans, IEP, probation/parole, etc).
- Complete intake paperwork, needed ROI’s, and informed consents as needed.
- Obtain CANS certification and complete CANS for each child as needed.
- Identify, advocate for, link, and coordinate needed services for child and their family, and help them access these services.
- Maintain client files and update client and program paperwork as needed.
- Communicate regularly with R&R Leads and other staff as needed.
- Attend supervision and other required meetings and trainings.
- Contact R&R Leads as needed for support.
- Represent the company as courteous, professional and ethical representatives.
- Ensure that personal transportation is functional and insured.
- Complete additional tasks as assigned by Community Coordinator.
- Maintain a working knowledge of assets, resources, and providers available in the community.

Qualifications Required:
- Must be 21 or older.
- Must have current CPR/First Aid, TCC, and CANS certifications.
- Experience working with youth or adolescents with an SED.
- Successfully pass the Idaho Department of Health and Welfare background check.
Education Required: Bachelor’s degree in Human Services or related field.
Language Skills: Ability to read, analyze, and interpret assessments and plans.

Work Environments:
Indoor office setting, client’s home, in businesses and agencies in the community, and in public settings. Optionally, walking or sitting outside with clients. Positive Connections Plus is a pet friendly facility and keeps cats as emotional support animals that are available for use in sessions. One on one, family, or group in person or via telehealth services. Ambient sound from clients in the children’s program and other office operations.
While performing the duties of this position, the employee may travel by automobile and be exposed to changing weather conditions.
Employees will be required to drive as needed for meetings and or visits to residential facilities. Employees will work in varied residential environments based on assigned caseloads.
Pay: Pay depends on experience.
